器質性精神障害に関する臨床的課題

Abstract

身体疾患が精神症状の原因となることは多いが，その精神症状のために精神障害と誤診されやすい．このような報告は古く よりなされてきたが未だに重要な課題であり続けている．特に原因が急性身体疾患であった場合は治療導入の遅れが生命予後 に大きな影響を及ぼす危険性があり，また慢性身体疾患の場合であっても不適切な治療や患者の誤った自己認識につながる恐 れがある．本稿では身体疾患と精神症状の関係について概説した上で，近年当院で経験した器質性精神障害として脳腫瘍，抗 NMDA（ N-methyl-D-aspartate）受容体抗体陽性脳炎，脳梗塞の3例を提示し，身体疾患を積極的に検索する事の必要性を改め て論じた．精神症状を呈した患者の診療を行う際には，論理上の先入観や検査・診察法の感度，精神障害診断における恣意的 判断の入りやすさなどに留意した適切な状態評価を行うことが重要であると考えられた．

Organic diseases often cause symptoms resembling mental disorders and tend to be mistakenly diagnosed as mental disorders．Such studies have been made，but it is still an important subject．In particular，in the case of psychiatric symptoms caused by acute organic diseases，delayed start of medical treatment is dangerous and sometimes affects life prognosis seriously．Even in the case of chronic organic diseases，a mistaken diagnosis may result in inappropriate treatment or mistaken self-recognition．Here，we give an outline of the relationship between organic diseases and psychiatric symptoms．We report three cases of organic mental disorders experienced in this hospital，one case each of brain tumor，anti-N-methyl-D-aspartate receptor antibody-positive encephalitis and brain infarction．Emphasis is laid on the necessity of a positive search for organic diseases．When seeing a patient showing psychiatric symptoms，it is necessary to keep in mind the possibility of logical prejudice and the sensitivity of examination and arbitrary judgment on the diagnosis of the mental disorder．
