静脈血栓塞栓症のチーム医療（第一報） ―京都市立病院VTE対策チームの活動と足跡

Abstract

静脈血栓塞栓症は，入院患者の在院死も来たし得る医療安全上留意すべき疾患であり，それを防ぐには一次予防・二次予防の取り組みが必要となる．京都市立病院において2009年から2025年まで行ってきた静脈血栓塞栓症に対する多職種チーム医療について，その活動内容と足跡を報告する．

Venous thromboembolism (VTE) is a disease that requires careful consideration in terms of medical safety, as it can lead to in-hospital death in hospitalized patients, and primary and secondary prevention efforts are necessary to prevent it. This paper reports on the activities and journey of the multidisciplinary team medical care for venous thromboembolism (VTE stewardship team) that was carried out at Kyoto City Hospital from 2009 to 2025.
