総合的・専門的な相談支援体制の強化のための予約システムの導入について

Abstract

地域包括ケアシステム構想を背景に，医療機関が地域に対し担う役割が拡充する中，当院では 3 部門で形成される「患者支 援センター」が，地域との窓口としての相談・連携機能を果たしている．医療ソーシャルワーカー（ medical social worker： MSW ）は，患者の多重複合的な社会課題に対し，医療・介護等の地域ネットワークを通じ，専門職として相談支援を行ってい る．地域住民のニーズと外来相談の重要性を，平成 28 年度から令和 3 年度の日報・月報データをもとに調査した結果，年間最 大約 1,700 件の相談，延べ約 3,800 件に対応していることが分かった．また，療養相談，経済相談，各種制度相談など専門的な 相談内容が大半を占めていた．結果を踏まえ，相談支援には早期発見，専門的な介入，地域との連携が不可欠と考え，外来患 者を対象に令和 4 年度「外来相談予約制」を導入するに至った．今後は，院内での外来相談予約制の浸透と地域連携強化の取 り組みに努めていく．

The medical facilities play a major role in regional care backed up by the Comprehensive Regional Care System in the back ground. At our hospital the Patient Support Center comprised of three sections serves as the contact counter and coordinator for consultation. The medical social workers (MSW) provide comprehensive professional consultation and support through the re gional network for medical care and nursing care to meet the patient’s multiple and compound social issues. A review of the daily reports and monthly data on the needs of the regional citizens and the importance of outpatient consulta tion from 2016 to 2021 revealed a total of approximately 1700 consultation cases each year and a total of 3800 consultation cases. Most of the cases required professional consultation such as consultation on recuperation, financial issues and various other sys tems. The results of the study suggested that early detection, professional intervention and regional cooperation are most import ant. Therefore, we introduced the Appointment System for Outpatient Consultation in 2022. We will work to establish the ap pointment system for outpatient consultation at our hospital and strengthen regional cooperation.
