造血細胞移植コーディネーターの役割と今後の課題

Abstract

造血幹細胞移植は，幹細胞を善意で提供するドナーが存在して初めて成立するという，通常の医療にはみられない特殊な医 療である．さらには，移植施設単体で移植を完結させることは出来ないため，日本骨髄バンク，さい帯血バンクをはじめとす る社会資源，関連機関，移植医療関係者等，多くの関係機関の協力により成り立っている． とりわけ血縁ドナー及び，他人であるドナーから提供された造血幹細胞を移植に用いる「同種移植」に関しては，移植実施 に至るまでに公平性や倫理性を担保することが，きわめて重要な課題であるとされている．今回，造血幹細胞移植の特性およ び課題について考察し，当院の移植コーディネーターが担う役割について報告する．

Hematopoietic stem cell transplantation (HSCT) is a special area of medicine that requires the presence of the stem cell donor. The transplantation cannot be completed without the cooperation of the Japan Marrow Donor Program, Cord Blood Bank, related societies, transplant medicine staff, and many related organizations. In allogeneic transplant, in which the hematopoietic stem cells obtained from the related donor or unrelated donor, an import ant issue is the fairness and ethics until the transplant. We discusses the peculiarity and issues of HSCT and the role of the trans plant coordinator.
