医薬品情報担当薬剤師と病棟薬剤師間カンファレンスが 医薬品副作用事例の収集に与える影響について

Abstract

医薬品の使用による副作用情報を医薬品医療機器総合機構（ Pharmaceuticals and Medical Devices Agency: PMDA ）に報 告することは医療関係者の責務だが，企業報告と比べ医療機関からの報告件数は低く推移している．京都市立病院薬剤科では 2012 年から薬剤師の病棟常駐を開始し，医薬品情報担当薬剤師と病棟薬剤師間でカンファレンスを実施してきたが，2016 年に カンファレンスの様式を見直し，副作用事例を共有する機会を設けることとした．今回カンファレンスにおいて副作用事例を 共有することが副作用事例の収集に与える影響について，薬剤師の病棟常駐開始前，開始後，カンファレンス様式変更後の各 4 年間において調査を行った．各期間における PMDA への副作用報告はそれぞれ 11 件，42 件，64 件であり，全報告者に占め る薬剤師の割合はそれぞれ 27 %，62 %，91 % であった．カンファレンスにおいて副作用情報を共有・検討することは薬剤師の 副作用報告に対する意識向上につながり，PMDA への副作用報告件数を増加させることが示唆された．

It is the duty of the medical staff to report the information of adverse effects caused by the use of medication to the Pharma ceuticals and Medical Devices Agency (PMDA). However, the number of reports from the medical institutions is much lower than the number from pharmaceutical companies. From 2012, the Department of Pharmacy has been assigning a pharmacist to each ward and gathered the ward pharmacist and the pharmacist in charge of drug information to hold conferences. In 2016, the confer ence style was changed to share the incidents of adverse effects. We conducted a study on the collection of incidents of adverse effects during each four years as follows: 1) before assigning a pharmacist to each ward, 2) after assigning a pharmacist to each ward, and 3) after changing the conference style. The number of reports of adverse effects to the PMDA during these periods was 11, 42 and 64, respectively, and the percentage of these cases reported by the pharmacists was 27 %, 62 %, and 91 %, respective ly. Sharing and reviewing the information of adverse effects at the conference raised the awareness of the pharmacist to report the incidents of adverse effects, which consequently led to an increase of the reports of adverse effects.
