救急室における臨床検査技師の役割と活動報告

Abstract

当院臨床検査技術科では，救急室業務支援を目的とし，2020 年 9 月より救急室に臨床検査技師を配置させる取り組みを開始 した．当初は週 3 回の午後のみの配置であったが，2021 年 10 月からは週 5 回の午後に救急室業務を行っている．臨床検査技師 が行う救急室業務は，超音波検査，心電図検査，採血・採血補助業務，患者移送などの看護診療補助業務が中心であり，その 件数は開始当初から徐々に増加している．2021 年 12 月に救急室業務に携わる医師・看護師を対象にアンケートを実施したとこ ろ「臨床検査技師の救急室配置についてどう思うか」に対する回答は「必要だと思う」が全体の約 80 % に及び，肯定的な意見 が多数であった．「役立っていると感じている内容について」に対する回答は，超音波検査が最も多く，次いで心電図検査，検 体搬送，採血・採血補助業務の順であった．2021 年 10 月 1 日施行の臨床検査技師などに関する法律の一部改訂により，検査技 師が採血時に静脈路確保を行うことが可能となった．それに伴い，病院の導入方針の決定次第であるが，今後より幅広い業務 支援につながる可能性がある．その他，人材育成についても積極的に取り組んでいく必要がある．

The purpose of our Clinical Testing Department is to support the emergency room. In September 2020, we began assigning a clinical technician to the emergency room. At first, the technician was assigned to do tasks in the emergency room in the after noon, three times a week. From May 2021, the technician was assigned to do tasks in the emergency room five times a week. The tasks of the clinical technician in the emergency room were mainly to support the nursing activities including ultrasonography, electrocardiography, blood sampling and patient transport. The number of cases handled increased since the start of the program. In December 2021, in response to the questionnaire on how they felt about the assignment of a clinical technician to the emergen cy room, 80 % of the doctors and nurses responded that the assignment was necessary. The tasks they felt most helpful were the assistance in electrocardiography, transport of specimens for tests, blood sampling and support of blood sampling, in this order. The amendment of the regulation issued on October 1, 2021 allows the clinical technician to secure the vein for blood sampling. As soon as the hospital provides the guidelines, further task share in various areas will become possible. Furthermore, it will be important to educate the staff aggressively.
