認知症サポートチームにおける薬剤師の関わり

Abstract

当院では，2018 年10 月より認知症ケア加算１の算定を開始し，薬剤師も認知症サポートチーム（dementia support team： DST）の一員として多職種カンファレンス及びラウンドに参画している．今回，当院DST における薬剤師の関わりの現状を 調査した．2019 年度上半期に，薬剤師は48 剤の処方提案を行なっており，そのうち29 剤（60％）がDST としての提案に反 映していた．薬剤師の提案内容は，不穏・せん妄状態や睡眠管理に対する薬剤の追加・増量，せん妄を惹起する可能性のある 薬剤や過鎮静を誘発する可能性のある薬剤の中止・減量であった．高齢者の薬物療法は注意すべき点が多く，DST への薬剤師 の参画は，有益であると思われた．

In our hospital, we started receiving dementia care incentive Type 1 in October 2018. Pharmacists also participate in multidisciplinary conferences and rounds as members of the dementia support team (DST). Here, we investigated the current status of pharmacist involvement in our DST. In the first half of 2019, pharmacists proposed 48 medicines, of which 29 (60%) were adopted in the proposal as DST. The pharmacists' proposals were to add or increase medicines for restlessness / delirium and sleep disturbance, and to discontinue or reduce medicines that may induce delirium or oversedation. There are many factors that require close attention to drug therapy for the elderly, and the participation of pharmacists in DST seems to be beneficial.
