難治性疼痛を有する終末期がん患者における症状緩和から自宅退院を可能にした一事例

Abstract

本研究は，難治性疼痛を有する終末期がん患者に対して症状緩和を図り，自宅退院が可能となった一事例から，自宅退院が できた要因や今後に必要なケアを明らかにすることを目的とした．患者の全身状態や言動，患者・家族の意向などを，カルテ から後ろ向きに抽出した．分析の結果，医療者の関わりから在宅移行を困難としている要因を取り除くことができていた．終 末期がん患者やその家族は，病状の変化や思いのゆらぎにより，療養場所に対する思いが変化するため，看護師は変化する思 いに寄り添いながら精神的支援を続けていくことが重要であると示唆された．

The purpose of this study was to identify the factors for a terminal cancer patient with intractable pain to return home and to clarify the care needed thereafter. I retrospectively extracted the patient’s systemic condition, behavior, and the wishes of the patient and family. As a result of the analysis, I was able to remove the obstacles for the patient’s return home. The results suggested the importance of the mental support by the nurse for the family coping with changes in feelings on where the patient should recuperate that accompany the changes in the patient’s feelings and health condition.
