がんゲノム医療と連携病院としての京都市立病院の役割

Abstract

Precision Medicine の一環として，がん患者の遺伝子情報に基づいた最適な治療薬を選ぶ「がんゲノム医療」である。がん組 織の遺伝子バリアントを網羅的に検査する「がん遺伝子パネル検査」が本邦でも利用可能となった．これらのうち主な4 つの 検査，OncoPrime，OncoGuideTM NCC オンコパネルシステム，FoundationOne?CDx がんゲノムプロファイル，Guardant 360 について，説明比較検討した．しかし最終的に有効と考えられる治療が受けられるのは全体の約10％に過ぎない点と，患 者本人だけにとどまらずその血縁者にも影響を及ぼす二次的所見Secondary Findings，SF と呼ばれる生殖細胞系列のバリアン トがみつかる可能性がある点，に特に注意を払う必要があり，その運用には十分なカウンセリングが不可欠である．当院も2018 年11 月からがんゲノム連携病院として，中核拠点病院である京都大学病院と協力して，「がん遺伝子パネル検査」を利用した がん治療に携わっている．

As a part of precision medicine, cancer genomic medicine where gene variants in solid cancer are comprehensively examined to find the therapeutic agent best for the patient, has become available in our country. Four leading genetic panel tests, Onco Prime, OncoGuide? NCC OncoPanel, Foundation One? CDx and Guardant 360, are reviewed and compared. Adequate counseling is essential for the operation because only 10% of all patients can receive effective treatment and there may be germline gene variants called “secondary findings” that affect not only the patient but also the blood relatives. As a cooperative hospital for cancer genomic medicine, in November 2018, Kyoto City Hospital started to put these cancer therapies in practice collaborating with the core base hospital, Kyoto University.
