患者尊厳を守る取り組み～身体抑制ゼロを目指した病棟ラウンドを通して～

Abstract

医療を提供する上で，患者の尊厳が守られているか確認し，倫理を維持していく必要性がある．患者尊厳を守り，生活力を 低下させない自立を支える看護の実践と尊厳を保障する仕組みの構築を目的として，今年度，看護部倫理委員会では病棟ラウ ンドを行い抑制について倫理的視点で検討されるよう支援した．またあわせて倫理カンファレンスのファシリテーターとなれ る人材育成，身体抑制率評価，身体抑制関連書類監査，記録監査を行なった．多職種が参加するカンファレンスでは，各専門 分野から情報提供を行ない，最小限の抑制を検討することで，専門性に特化したカンファレンスが行えていた．抑制実施率は 減少しているが，転倒転落件数に関しては大きく変化はないことから安全を保ちながら最小限の抑制を検討できていることが データから推測できる．抑制率の減少は継続した各ラダーOJT（ On-the-Job Training ）による倫理教育，定期的な倫理研修の 実施から，スタッフの倫理意識が高くなっている結果と考えられる．多職種で身体抑制カンファレンスを実施する事は，様々 な専門性から身体抑制解除のための支援策の幅を広げ，組織的に患者の尊厳を守ることに繋がる．患者の尊厳を守るために倫 理方針にそった医療提供を行ない，普遍的な基準として倫理感を養う教育の場が必要である．

When providing medical care it is important to confirm that the patient’s dignity is protected and ethics is maintained. The Ethics Committee of the Department of Nursing conducted a ward round on the physical restraints from the ethical view point, with the aim to establish a system in nursing practice for supporting the patient’s independence by preventing loss of vitality and for guarantee of the patient’s dignity. In addition, we inspected the training of persons to become facilitators of the ethical conference, evaluated the rate of physical restraints and audited the documents concerning physical restraints. At the conference consisting of participants in multidisciplinary occupations, information was provided from each specialist’s field, and the issue on reducing physical restraints to a minimum could be discussed efficiently. The decrease in the rate of restraints without much change in the incidence of falls indicates that the physical restraints could be reduced without jeopardizing the safety of the patient. The lower rate of restraints was considered to be the result of the higher ethical consciousness of the staff attained by continued periodic ethics training and on-the-job training following the nursing clinical ladder program. The conference on physical restraints with participants from multidisciplinary fields will broaden the range of measures to reduce the physical restraints and systematically protect the patient’s dignity. A training field is necessary to nurture the ethical concern as a universal standard to provide medical care along the ethical guidelines that will help protect the patient’s dignity.
