| 抄録: | 今回連携室看護師として在宅での看取りを希望する患者・家族の自己決定を支え住み慣れた自宅で終末期を迎えられるよう退院支援・調整で介入した症例を経験した．ターミナルステージの患者では，スムーズな在宅療養への移行が望まれると考えた．
Herein, I report the experience of a case in which I intervened as a community health care nurse to help the team support the decision made by the patient and family to receive terminal care at home. I concluded that the terminal care patient should receive support for smooth transfer to receive care at home． |
