当院における外来化学療法センターこの5年間の取り組みについて

Abstract

京都市立病院は2007年1月に地域がん診療連携拠点病院に認定され，外来化学療法センターを設置した．2013年には新棟 開設とともに14床となり，現在さらに16床に拡大，15科より年間350人を超える利用数となっている．利用者全例に対する 医師，薬剤師，看護師を中心としたチーム医療によるがん患者指導により，B型肝炎スクリーニングとフォローアップ，有害 事象の確認と，服薬指導を徹底して施行している．また2015年度からは需要の増加に対応するため休日開院を開始，2017年度 からは認定看護師による静脈確保も開始した．今後，地域との連携を充実させ，更なるチーム医療の充実を図りたい．

Kyoto City Hospital was certified in January，2007 as a local cancer hospital and installed a chemotherapy center for outpatients． When a new center opened in 2013，14 beds were added and in 2016， the number was increased to 16 beds． Now more than 350 patients use the center． The guidance for cancer patients with team care led by a doctor， a pharmacist and nurse has been provided from 2010 including screening of HBV status and follow up， confirmation of the adverse events and instructions to increase patient compliance． We also started keeping the center open on holidays from 2015． Authorized nurses started to perform intravenous access in 2017 and preventive measures for exposure of antineoplastic drugs were advanced． We plan to expand the cooperation with the local community to improve further team care in the future．
