脳卒中センターにおけるリハビリテーション科の取り組み : ウォーキングADLカンファレンスの実績報告

Abstract

【緒言】当院脳卒中センターにて平成28年5月よりウォーキングADLカンファレンスを開始し，早期の適切なリハビリテーション介入を促している．その実績を報告する．【対象と方法】平成27年および平成28年の各年8月1日から10月31日までに当院脳卒中センターに入院した脳卒中患者それぞれ68名，70名を対象とし，リハビリテーションオーダー，実施単位数について，カンファレンス開始前後での比較を行った．【結果】脳卒中入院患者に対するリハビリテーションオーダーの割合は平成27年76.4％から平成28年83.3％へと増加した．また，脳血管リハビリテーション総実施単位数は1,950単位から2,577単位へと有意に増加し（p<0.05），患者1人あたりの1日の実施単位数も1.37±0.80単位から1.77±0.85単位へと有意に増加した（p<0.01）．【結語】ウォーキングADLカンファレンスにより，リハビリテーション対象者数および実施量が増加し，脳卒中リハビリテーションの充実をはかることができた．

[Introduction] In the Stroke Center of our hospital，the walking conference on activities of daily living (walking ADL conference) began in May，2016．The conference has helped promote early rehabilitation for stroke patients．We analyzed the outcome of our conference．[Objectives and methods] The subjects were patients hospitalized in our Stroke Center during the period August 1 to October 31 in 2015 and 2016, i.e., 68 and 70 patients, respectively．The number of rehabilitation orders and the exercise time were compared．[Results] The number of rehabilitation orders for hospitalized stroke patients increased from 76.4% in 2015 to 83.3% in 2016．The total amount of rehabilitation exercise time increased significantly from 1,950 units in 2015 to 2,577 units in 2016 (p<0.05)and the exercise time for each patient per day also increased significantly from 1.37±0.80 units to 1.77±0.85 units (p<0.01)． [Conclusion] The number of rehabilitation orders and the exercise time were increased．The walking ADL conference recommends more rehabilitation exercise for stroke patients.
