フットケア外来における足部の実態調査 : 当院のフットケアの動向も踏まえ

Abstract

フットケア外来では，外来通院中の糖尿病患者を対象に，足病変の予防を目的とした指導・管理を行っている．昨年度，糖尿病診療の質を向上する目的で，糖尿病神経障害がある患者を中心に積極的にフットケア外来を紹介したところ，受診者数が増加した．そこで，フットケア外来を受診した患者の足部のトラブルの実態調査を行った．その結果，対象患者114名中，皮膚症状は乾燥が79名，胼胝が32名，浮腫が17名の順に多かった．爪症状は，肥厚爪が54名，巻き爪が25名，陥入爪4名であった．「立つ」「歩く」という基本的な動作の要である足は転倒予防との関連も深く，当院でも昨年度から転倒予防の取り組みの1つとして，糖尿病以外の患者についてもフットケアを行っている．患者の自立支援を促す看護ケアのひとつとして，足のケアが大切であるということを広く周知していきたい．

Our outpatient foot care clinic conducts guidance and management of foot care to prevent foot lesions in outpatients with diabetes．Last year，patients with diabetes especially those with nerve disorders were referred to the outpatient foot care clinic to improve the quality of the treatment of diabetes．Consequently，the number of patients visiting the outpatient foot care clinic increased，and a survey was conducted on the foot condition of these patients．Among the 114 patients studied, the number of patients with dry skin，callus and edema was 79，32 and 17，respectively．The nail symptoms were thickened nail，deformed nail and ingrown nail in 54，25 and 4 patients，respectively．The basic function of the foot is to “stand and walk”, which relates to prevention of falls．Thus starting last year，even the patients without diabetes have been receiving foot care for the purpose of preventing falls．It is necessary to spread the word that foot care is important as a part of nursing care to further support the patient's independency.
