抄録: 当院では新型コロナウイルス感染症患者の受け入れを 2020 年 1 月から開始し，看護師は未知のウイルスに不安を抱きながら
も感染症病棟を預かる看護師として奮闘してきた．また，院内クラスターを乗り越え，COVID-19 流行の波に連動した受け入れ
患者数の増減と業務内容の変動を余儀なくされた中で，病棟管理者は様々なストレスを抱える看護師のモチベーションを維持
するために部署看護師を巻き込んで教育計画を立案して取り組んだ．このことにより，看護師の意識の変化等，評価できる成
果が得られた．; We started accepting COVID-19 patients in January 2020. As members of the infectious disease ward, the nurses coped with
the unknown virus with anxiety. The staff had to handle in-hospital clusters and adapt to the surge of the patients fluctuating in
number with the pandemic wave, while adjusting their shift and workload according to the situation. The ward managers planned
the education schedule with the participation of the ward nurses, in order to maintain the motivation of the nurses who were under
various stresses. 抄録: COVID-19 患者は若年層であっても発熱や低酸素状態の影響もあり転倒リスクが高い．一般病棟では，患者のベッドサイドで
実際の動きなどを確認しながら多職種で転倒防止対策を検討することが効果的であるとされているが，感染症による隔離状態
ではこのような対応は困難である．そのため，タブレット端末を用いたビデオ通話を活用し，多職種による患者参画型転倒防
止カンファレンスを行った．病室内の看護師と患者が，スタッフステーションに待機している医療者とともに，自宅での様子
や動線を参考に病室内のレイアウトや動線を検討するという方法である．その結果，転倒件数が減少し，カンファレンスが，転
倒転落の予防対策として有効であると考えられた．; The risk of falls is high even in young COVID-19 patients with a high fever and low oxygen saturation. In the general hospital
ward, the most effective way to decide the countermeasures for fall prevention would be to confirm the actual line of flow with the
multidisciplinary staff at the actual bed side. However, this is difficult when the patient has a contagious disease and is in an iso lated condition. Therefore, we held a conference on fall prevention with the participation of the multidisciplinary staff and patient
using video calls and tablet computers. Taking into consideration the line of flow at the patient’s home, we decided the layout of
the hospital room with the participation of the patient and nurse in the hospital room, and the medical staff at the staff station. As a
result, the incidence of falls decreased. 抄録: 近年，外来化学療法患者が増えている．化学療法中の体重減少や栄養障害は治療継続にも影響し，患者個々の状態にあわせ
た栄養管理が求められるが，当院において入院に比べて外来での栄養食事指導の機会は少ない．外来化学療法センターでの栄
養指導を拡充するため，多職種で連携し栄養指導の必要な患者を抽出し栄養指導介入へつなげる体制を構築した．; Patients on chemotherapy at the outpatient clinic are on the increase. Management of nutrition according to the condition of
each patient is important since weight loss and malnutrition during chemotherapy affect the continuation of therapy. However, as
compared to the inpatients, the outpatients have less opportunity to receive nutritional guidance. In order to provide better nutri tional guidance at the outpatient chemotherapy center, we developed a system to support nutritional intervention by extracting the
patients requiring nutritional guidance with the cooperation of a multidisciplinary team.2021-12-31T15:00:00Z
